How To Apply:
If there is an online application available, you may use the online form. Be careful to supply all necessary information. If you have supporting documents (letters, images, etc.) to go along with the application, be sure to bring these to the Chamber office before the deadline.
If there is no online form available, you must print, complete and submit the pdf version. Be sure to download and complete a cover page with each submission. All forms and supporting documents must be delivered to the Chamber offices prior to the deadline.

| Agriculture / Agri Business Award This award recognizes any individual, group, organization, or business where the principles of running a successful agricultural or related operation are applied and result in a thriving and environmentally friendly endeavor. |

| Business of the Year Award (15 or more employees) This award is presented to a company that has had an impact on the Centre Wellington community and has 15 or more employees. This company has demonstrated growth, sales, leadership, an active involvement in the community, good business citizenship, sound employee relations, safety in the workplace, environmental awareness, introduction of a new product or idea, expansion or exportation of goods or services or the turnaround of a failing business. |

| Business of the Year Award (under 15 employees) This award is presented to a company that has had an impact on the Centre Wellington community and has fewer than 15 employees. | Business Beautification Award This award is presented to a business or industry that has taken great pride and ownership of their community corporate image through the exterior maintenance and upkeep of flowerbeds, grounds and landscaping. Supporting photographs must be attached to this application. |

| Citizen of the Year This award is presented to an individual who makes an outstanding contribution to our community, with no expectation of personal gain. Unselfish volunteering of personal time and talent towards the betterment of our community are the hallmarks of this award. |

| Community Award of Merit This award is presented to an individual, group, organization or business that has displayed hard work and dedication to bettering our community. A nominee could be a group of employees who have helped raise money for a special cause. A nominee could be an organization whose sole purpose is to assist others or better our community as a whole. A nominee could also be a business that has supported our community through donations, fundraising or special events. |

| Corporate Citizen of the Year Award This award is presented to a business which actively works towards making our community a more friendly, safe and pleasurable place to live. The Corporate Citizen of the Year Award identifies those who have a concern for the needs of others, going above and beyond the call of duty with little or no concern for financial gain. |

| Cultural / Arts Award of Merit This award recognizes any individual, group, organization or business for a significant contribution to Centre Wellington’s Cultural environment through distinguished service or artistic achievement. |

| Customer Service Excellence Award This award is presented to a business that has provided exceptional customer service by consistently offering a high quality of service to all customers or by going above and beyond the call of duty to provide service to a customer in extenuating circumstances. This nomination must be third party (i.e. no self-nominations). |

| Environmental Excellence Award This award is presented to a business, person or organization that has demonstrated a commitment to the environment. |

| Sport Achievement Award This award is presented to a Centre Wellington athlete or team, but may include competitors from outside Centre Wellington. Local athletes who participate in their particular sport outside of Centre Wellington are not eligible to be nominated. Nominations may include those teams/athletes which are amateur or professional in their given field of athletics. In the case of a coach or team executive member, they must have a proven dedication to their sport that goes above and beyond what is called for in that role, and have also assisted with raising the profile of their organization within Centre Wellington. |

| Tourism Impact Award This award is presented to a business attraction, event, group, individual, or organization that has influenced the growth and development of tourism within the community of Centre Wellington. |

| Youth Citizen of the Year The Township of Centre Wellington Parks and Recreation Department sponsors The Alvin Shortt Award, recognizing outstanding youth (20 and under) within the Township of Centre Wellington. The individual must maintain a high level of proficiency in their activity while, at the same time, volunteering and giving back to the community of Centre Wellington. This award recognizes excellence in youth in Centre Wellington with leadership, sportsmanship, education, and work ethic all being major factors in this award. The award is open to all Centre Wellington community groups, (including secondary schools serving the youth of Centre Wellington), sport groups, and organizations that wish to recognize the outstanding achievements of a local youth by nominating him or her. The group’s contact person should be clearly highlighted on the nomination form, which will outline the applicant’s achievements and Centre Wellington commitments. |

| Youth Entrepreneur of the Year This award is presented to an individual between the ages of eighteen and twenty-nine who has distinguished himself or herself by having emonstrated outstanding business sense, an innovative spirit, an energetic profile and good community involvement. |
